Birthday Book Club Form

FWCD's Birthday Book Club is a great way to celebrate a student's birthday by donating a book to the Lower School Library. The book you purchase will include a bookplate dedicated to the birthday student. The cost is $30.00 and may be paid by credit card or charged to the student's account.

Your child will choose from a preselected group of new books. Caregivers are welcome to come and assist in the book choice. After the dedication plate is added, the birthday book will be checked out to the student for the whole family to enjoy. Upon the book's return to the LS Library, it will be placed in the LS Library's collection.
